Note: If you choose to pay via credit card I need the address at which you receive your credit card bill. If that address is different from your shipping address I need an explanation for that.

Order Form Terms Defined:

Quantity = how many of the item you want.

Item = type of item (painting, print, book).

Description = image desired / title (rabbit, seated cat, blue iris / gyotaku primer).

Unit Price = cost of one item.

Total = cost of one item times the quantity desired.

Sub Total = add up the total column.

Shipping = $10. total for matted paintings/prints; $20. total for framed paintings/prints; $5. total for one or more books...

Note: shipping charge is per address being shipped to. So it would be a total of $10. for three matted prints if all are being shipped together to one address.

Grand Total = the amount you are enclosing as payment for your order.
